Many primary-care physicians are no match for determined adolescents who are trying to cheat on urine drug tests, researchers here said.

More than 70% of 359 primary-care physicians surveyed did not follow effective procedures to prevent switching, diluting, or tampering with urine samples, reported Sharon Levy, M.D., M.P.H, of Children's Hospital here and colleagues in the February issue of the Archives of Pediatric and Adolescent Medicine.

A few tips:

  • Check the temperature of the sample immediately
  • Put blue dye in "standing water" in the bathroom (i.e., the toilet)
  • Have a staff member watch the patient while urinating (that will be popular!)
  • Check the sample for creatinine or specific gravity to reveal dilution
  • Remember that Ecstasy, oxycodone, and nitrous oxide are not detected by the usual screens

Now, if only the kids could put their ingenuity and technical skill to something useful -- like rigging up their doctors' offices with electronic health records.
