In Part I of my podcast interview with Dr. Paul Grundy, Director of Healthcare Transformation at IBM and President of the Patient-Centered Primary Care Collaborative (PCPCC) discussed principles behind the Patient Centered Medical Home, IBM's interest in the topic, and how Denmark and Sweden were approaching primary care.
